*Only a parent or legal guardian of a player-applicant is authorized to submit an application.

Each application will undergo meticulous evaluation, with invitations extended to chosen candidates at the discretion of the Academy. These invitations will be issued based on the Academy’s schedule and requirements, as well as the applicant’s demonstrated capability to compete at the suitable level. Please be aware that participation in a trial is not guaranteed for all applicants.

Medical Release Form

I, the undersigned parent/guardian of the participant, grant permission for necessary medical treatment to be administered to my child in case of an emergency during soccer activities. I understand the inherent risks associated with soccer and voluntarily assume all risks of injury or harm that may arise from participation. I release and hold harmless SC Pro Soccer Academy, its coaches, officials, and volunteers from any and all claims, liabilities, or expenses resulting from my child’s participation, including any injuries sustained, whether caused by negligence or otherwise.

Photo Release Form

SC Pro Soccer Academy has my permission to use my and/or my child(ren) photograph publicly to promote the club. I understand that the images may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me and/or my child (ren) for any reason of such use.

Inclement Weather

While we strive to provide the best soccer tryouts regardless of the season, severe weather conditions, particularly during the winter months, may impact our outdoor sessions. In the event of severe weather that prevents us from running a tryout as scheduled (and if indoor accommodations are unavailable), we may have to cancel the affected session. We appreciate your understanding and flexibility as we prioritize safety while maintaining our commitment to player development. **Please check your emails the night before/morning of the event for any cancellations and updates.
